Job summary

Mosaic Life Care is seeking an Inpatient Coder II to assign ICD-10-CM/PCS codes for acute inpatient, rehab, swing bed, and LTACH services. You will query providers for documentation, resolve edits, and mentor coders while ensuring compliance and coding accuracy.

Responsibilities include DRG assignment, education delivery, and collaboration with clinical teams to support patient care quality and documentation integrity. This role emphasizes professional development and coding quality goals.

Qualifications

• Must have coding education with knowledge of medical terminology, pharmacology and anatomy, query guidelines - Required
• Associate's Degree - Health Information Management / Medical Records - Required OR
• Bachelor's Degree - Health Information Management / Medical Records - Preferred

Responsibilities

• Codes complex diseases, procedures and diagnoses using ICD-10-CM/PCS according to guidelines.
• Queries providers following Official Query Guidelines for documentation quality.
• Assigns MS-DRG and APR-DRG per regulatory guidelines.
• Mentors and trains coders and supports education.
• Ensures data accuracy for regulatory reporting and resolves edits.
• Works with cross-functional teams to ensure documentation accuracy.
