Job summary

Mosaic Life Care is seeking an Inpatient Coder II to accurately assign ICD-10-CM and ICD-10-PCS codes for acute care inpatient, rehab, swing bed and LTACH services. The role emphasizes query-driven documentation improvement and collaboration with clinical teams.

You will review records, educate coders, mentor staff, and ensure coding quality and regulatory compliance, with a strong foundation in pathology, anatomy, pharmacology and coding guidelines.

Qualifications

• Must have coding education with knowledge of medical terminology, pharmacology and anatomy.
• Associate's Degree - Health Information Management / Medical Records - Required
• Bachelor's Degree - Health Information Management / Medical Records - Preferred

Responsibilities

• Codes ICD-10-CM/PCS for inpatient services (acute care, rehab, swing bed, LTACH).
• Queries providers per Official Query Guidelines to improve documentation.
• Assigns MS-DRG and APR-DRG per regulatory guidelines.
• Mentors and trains coders; participates in education opportunities.
• Ensures data accuracy for HIDI and other regulatory agencies.
