UnitedHealth Group is seeking an HSPC Afterhours Triage Nurse - Remote to coordinate after-hours hospice care and ensure continuity of care when centers are closed. Qualifications

• Active Registered Nurse (RN) license in state of practice.
• 1+ year of RN experience.
• Ability to conduct telephonic clinical assessments with nursing judgment.
• Proficiency with electronic health records (EHR), email, internet, and MS Office.
• Willingness to obtain additional RN licenses in other states as required.
• Reliable high-speed internet for remote work.

Responsibilities

• Respond to after-hours hospice calls, assess needs, and determine appropriate next steps.
• Conduct telephonic patient, caregiver, environmental, and psychosocial assessments using the plan of care.
• Educate patients and caregivers on hospice guidance and symptom management.
• Coordinate with on-call staff, pharmacies, DME vendors, and agencies to facilitate visits and follow-up.
• Document assessments and care coordination activities in real time while following policies.
