Job summary

UnitedHealth Group seeks an HSPC Afterhours Triage Nurse to coordinate afterhours hospice care and provide telephonic assessments for patients and caregivers. You will educate families, guide symptom management, and coordinate with on-call teams and pharmacies while documenting care in real time.

The role requires an active RN license, at least 1 year of nursing experience, and strong technology skills. Remote work is available with travel as needed.

Qualifications

• Current, unencumbered RN license in state(s) of practice.
• 1+ year of RN experience.
• Proficiency with electronic health records and common software.
• Ability to conduct phone-based clinical assessments.
• Willingness to obtain additional RN licenses in other states (reimbursed).

Responsibilities

• Respond to afterhours hospice calls and assess needs using nursing judgment.
• Conduct telephonic assessments of patient, caregiver, and environment.
• Provide education on hospice care and symptom management.
• Coordinate with on-call staff, pharmacies, and vendor services.
• Document assessments and care activities accurately and promptly.
