Job summary

TRU Community Care is seeking a phone triage RN to support afterhours triage. You will handle calls, assess patient needs, and coordinate with visit nurses to determine the best care plan. Remote work with minimal in-person requirements, requiring residency in CO/FL/GA and an RN license.

Ideal candidates have at least 1 year of RN experience, hospice/home health or PACE is preferred, and must pass background checks and immunization requirements.

Qualifications

• Must have a secure HIPAA-compliant workspace with reliable internet.
• Current RN license in Colorado or compact license is required.
• Residency in Colorado, Florida, or Georgia is required.
• Minimum of one year RN experience; hospice/home health/PACE preferred.

Responsibilities

• Answer calls on the afterhours triage line and support the Care Team.
• Assess patient needs using triage protocols and provide guidance.
• Collaborate with visit nurses to determine clinical actions.
• Direct patients to ER, urgent care, or arrange in-home visits as needed.
• Operate within HIPAA and agency guidelines during triage.
