UnitedHealth Group is seeking a highly capable HSPC Afterhours Triage Nurse to coordinate hospice care after hours. You will perform telephonic assessments, educate patients and caregivers, and partner with on-call staff to arrange visits and ensure continuity of care.

The role emphasizes professional nursing judgment, empathy in high-stress situations, and strong communication while supporting patients in their homes, with travel as required and a robust benefits package.

Competenze

• Active, unencumbered RN license in state(s) of practice (NLC where applicable).
• Minimum 1+ year RN experience.
• Comfort with technology, including EHR, email, internet, and MS Office.

Mansioni

• Respond to afterhours hospice calls using nursing judgment to determine next steps.
• Conduct telephonic assessments of patient, caregiver, environment, and psychosocial factors per plan of care.
• Provide education, hospice guidance, and symptom management aligned with standards.
• Coordinate with on-call staff, pharmacies, DME, and other agencies to arrange visits and ensure continuity of care.
• Document assessments and care activities accurately and in real time per policies.
• Perform other related duties as assigned.
