UnitedHealth Group is seeking a Hospice RN On-Call to provide after-hours clinical support, triage patient needs, and coordinate care for continuity and high-quality outcomes. The role combines remote and field-based nursing, assessing conditions and collaborating with interdisciplinary teams.

The position requires an active RN license, CPR certification, and at least 1 year of nursing experience, preferably in hospice or home health.

Qualificações

• Current RN license in the state(s) assigned.
• CPR certification current and active.
• At least 1 year of nursing experience, preferably hospice/home health/acute care.
• Valid driver's license, reliable transportation and liability insurance.

Responsabilidades

• Provide after-hours on-call nursing support and triage patient needs.
• Assess patient conditions via phone or in-person visits and escalate when needed.
• Coordinate with physicians, care teams and caregivers to ensure timely care.
• Educate patients and caregivers on symptom management and care plans.
• Document all patient interactions and clinical decisions accurately.
