Job summary

Moments Hospice seeks an experienced RN for a fully remote Triage role. You will provide after-hours clinical triage, assess patient needs, and coordinate care using the nursing process and Telecare protocols.

The position is ideal for an independent, compassionate hospice professional who can work remotely and support families remotely. You will document interactions in the EMR, coordinate with physicians, facilities, and internal teams, and ensure high-quality patient outcomes.

Qualifications

• Active RN license with no current or pending restrictions.
• Hospice experience is required.
• Home Care Home Base (HCHB) experience is required.

Responsibilities

• Provide after-hours triage, clinical assessment, and interventions via phone.
• Support patients and families with symptom management and end-of-life care needs.
• Coordinate care by collaborating with physicians, facilities, and internal teams.
• Dispatch visits and coordinate services with vendors as needed.
• Deliver clear, compassionate education and support to families.
• Accurately document all interactions, care plans, and outcomes in the EMR.
• Review team documentation for accuracy and compliance.
• Partner with Telecare staff to ensure positive patient outcomes.
