Job summary

UnitedHealth Group is seeking an Afterhours Hospice Triage Nurse to coordinate non-urgent hospice care needs after hours. You will respond to calls, perform telephonic assessments, and guide care plans using established nursing judgment and the EHR.

The role requires an active RN license and 1+ year of RN experience, with ability to travel as needed. A remote work setup and comprehensive benefits are provided.

Qualifications

• Current, unencumbered RN license in the state(s) of practice.
• 1+ years of experience as a Registered Nurse.
• Knowledge of terminally ill patient and caregiver needs; strong nursing judgment by telephone.

Responsibilities

• Respond to afterhours hospice calls and determine care/support steps.
• Perform telephonic nursing assessments using plans of care and EHR.
• Educate patients and caregivers; provide symptom management guidance.
• Coordinate with on-call staff, pharmacies, DME vendors and others to arrange visits and follow-up.
• Document assessments and interventions accurately and in real time per policies.
• Perform other related duties as assigned.
