UnitedHealth Group is seeking an HSPC Afterhours Triage Nurse (PT) to coordinate hospice care after hours and ensure continuity of care when centers are closed. You will respond to calls, perform skilled telephonic assessments, educate patients and caregivers, and coordinate follow-up using established plans of care.

You will work with on-call staff and vendors, document interventions in real time, and travel as required.

Competenze

• Active RN license in applicable states incl. NLC eligibility where applicable.
• Minimum 1 year of RN experience.
• Experience with telephonic or remote triage.
• Proficient with EHR, email, and MS Office.

• Respond to afterhours hospice calls, assess needs, and determine next steps.
• Conduct telephonic assessments of patient, caregiver, and environment.
• Provide education, hospice guidance, and symptom management.
