Chapters Health System, Inc. is seeking a Resource Nights & Weekend (ReNW) Nurse Practitioner to provide fully remote virtual serious illness care management across our continuum, including home-based and hospital settings.

The NP will work with the medical staff and interdisciplinary team to assess, plan, and manage patient care. You will contribute to discharge planning, medication management, and patient education while maintaining HIPAA compliance and leveraging EMR and electronic

Qualificações

Minimum of 3 years of recent clinical experience in hospice, palliative care, or acute inpatient care.
Masters or Doctoral degree in Nursing from an accredited Family or Adult NP program.
Must hold national certification, licensure, and prescriptive authority for advanced practice nursing in home state and other states where care is provided.
Basic Life Support (BLS) certification and compact RN license in home state by date of hire.

Responsabilidades

Provide fully remote virtual serious illness care management to patients and families.
Consult with medical staff and IDT members to ensure high-quality patient care.
Conduct virtual assessments of patient/family needs and plan of care.
Identify, manage, order, and prescribe medications and equipment as appropriate.
Maintain EMR access and document interactions in a timely manner.
Collaborate with primary team to ensure continuity of care and discharge planning.
