Job summary

The Resource Nights & Weekend (ReNW) Nurse Practitioner (NP) at Chapters Health System provides fully remote virtual serious illness care management to patients and families across our continuum, including hospice, palliative care, and related programs.

Must have a Masters or Doctoral degree in Nursing (Family or Adult NP) with national certification and licensure, plus BLS and RN licensure in home state. Scheduling may include nights and weekends.

Qualifications

• Must hold a Masters or Doctoral degree in Nursing from an accredited Family or Adult NP program.
• National certification for advanced nursing practice in home state and additional states.

Responsibilities

• Provide fully remote virtual serious illness care management to patients and families.
• Consult with medical staff to ensure optimal patient care.
• Perform virtual assessments of patient/family needs and adjust care plans.
• Prescribe medications and equipment as appropriate within scope.
• Maintain EMR access and document interactions promptly.
• Collaborate with the primary team and support discharge planning and QA activities.
