Job summary

Chapters Health System, Inc. is seeking a remote Nurse Practitioner to provide fully remote serious illness care management across home, facility, and inpatient settings.

The NP will work with a multidisciplinary team to coordinate hospice, palliative care, and related services, ensuring high-quality patient and family care. Requirements include a Masters/Doctorate in Nursing (Family or Adult NP), national cert, licensure and prescriptive authority, and BLS.

Qualifications

• Minimum 3 years of recent clinical experience in hospice, palliative care, or acute inpatient care.
• Master's or Doctoral degree in Family or Adult Nurse Practitioner.
• National certification, licensure, and prescriptive authority required in home state and others.

Responsibilities

• Provide fully remote virtual serious illness care management to patients and families.
• Consult with medical staff to ensure optimal patient care in an efficient manner.
• Perform virtual assessment of patient/family/caregiver needs and manage meds/equipment.
• Document interactions in the EMR and communicate with the primary team.
• Participate in QAPI activities and stay current with hospice/palliative care practices.
