Job summary

Chapters Health System, Inc. is seeking a seasoned Nurse Practitioner to provide fully remote, high-quality hospice and palliative care management to patients and families across our care continuum.

You will collaborate with an interdisciplinary team, assess patient needs virtually, prescribe medications as appropriate, and document in the EMR while supporting discharge planning and ongoing quality improvement activities. This role requires flexibility for varied shifts.

Qualifications

• Minimum of 3 years of recent clinical experience including hospice, palliative care, or acute inpatient care.
• Masters or Doctors degree in Nursing from an accredited Family or Adult Nurse Practitioner program.
• National certification, licensure, and prescriptive authority for advanced nursing practice in home state and in additional state(s).
• Must hold and maintain Basic Life Support certificate and compact RN license in home state by date of hire.
• Shifts may vary and can include nights, evenings, weekends, and/or holidays.

Responsibilities

• Provide fully remote virtual serious illness care management to patients and families across our continuum of care.
• Consult with medical staff to ensure optimal patient care.
• Perform virtual assessment of patient/family/caregiver needs.
• Identify, manage, order, and prescribes medication and equipment as appropriate.
• Document interactions in the EMR in a timely manner.
• Communicate with the primary team to ensure continuity of care.
• Support discharge planning including prescribing medications and discharge orders and instructions.
• Participate in QAPI activities to improve care practices and regulatory compliance.
• Maintain knowledge of hospice and palliative medicine trends.
