Job summary

Chapters Health System, Inc. is seeking a Nurse Practitioner with extensive hospice and palliative care experience to join our team in a remote capacity.

You will deliver virtual serious illness care management, coordinate with the IDT, and support care plans for patients across home, facility, and inpatient settings. The ideal candidate will hold an NP degree (Family or Adult), national certification, and prescriptive authority, with BLS and RN licensure current by hire.

Qualifications

• Minimum 3 years of recent clinical experience including hospice, palliative care, and/or acute inpatient care.
• Master's or Doctorate in Nursing (Family or Adult NP).
• Must hold national certification, licensure, and prescriptive authority for advanced nursing practice in home state and others.
• Must have Basic Life Support certification and compact RN license in home state by hire date.
• Shifts may vary including nights, evenings, weekends, and holidays.

Responsibilities

• Provide remote virtual serious illness care management to patients and families across the continuum of care.
• Consult with medical staff and IDT members to ensure optimal patient care.
• Perform virtual assessment of patient/family/caregiver needs.
