Chapters Health System, Inc. seeks an experienced Nurse Practitioner to deliver remote serious illness care management across our continuum for adults and pediatrics.

The role emphasizes coordination with the care team, medication management, and ensuring compliance with regulatory standards. Applicants must have 3+ years of clinical experience in hospice/palliative or acute inpatient care, a NP degree, national certification, and BLS.

Qualifications

• Minimum of 3 years of recent clinical experience, including hospice/palliative or acute inpatient care.
• Master's or Doctoral degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
• Must hold national certification, licensure, and prescriptive authority for advanced nursing practice in home state and other states served.
• Must hold and maintain Basic Life Support certificate and compact RN license by hire date.

Responsibilities

• Provides fully remote virtual serious illness care management to patients and families across our continuum of care.
• Consults with medical staff and IDT to ensure optimal patient care.
• Performs virtual assessment of patient/family/caregiver needs.
• Identifies, manages, prescribes medications and equipment as appropriate.
• Maintains EMR access and documents interactions in a timely manner.
• Supports discharge planning including prescribing medications and discharge instructions.
• Participates in QAPI activities to improve care practices and compliance.
