Job summary

Chapters Health System is seeking a Remote Nights & Weekend NP to provide fully remote serious illness care management across our continuum, including hospice, palliative care, and related programs. The role supports home, facility, and inpatient settings with flexible shifts.

Candidates must have 3+ years of hospice/palliative/acute inpatient experience, a Family/Adult NP degree, national certification, and licensure with prescriptive authority.

Qualifications

• Minimum 3 years of recent hospice/palliative/acute inpatient experience.
• Master's or Doctorate in Nursing from an accredited Family or Adult NP program.
• NP must hold national certification, licensure, and prescriptive authority in home state and other states served.

Responsibilities

• Provide remote serious illness care management to patients and families.
• Consult with medical staff to ensure optimal patient care.
• Perform virtual assessments of patient/family needs.
• Identify, manage, prescribe medications and equipment as allowed.
• Maintain EMR access and timely documentation.
• Collaborate with the primary team for continuity of care.
• Participate in QAPI activities and quality improvement.
