Qualifications

• Must have minimum of 3 years of recent clinical experience in hospice, palliative care, or acute inpatient care.
• Masters or Doctors degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
• Must hold national certification, licensure, and prescriptive authority for advanced nursing practice in home state and other states where care is provided.
• Must hold and maintain Basic Life Support certificate and compact RN license by date of hire.
• Shifts may include nights, evenings, weekends, and/or holidays.

Responsibilities

• Provides fully remote virtual serious illness care management to patients and families throughout the continuum of care.
• Consults with medical staff to ensure optimal patient care.
• Performs virtual assessment of patient/family/caregiver needs.
• Identifies, manages, orders, and prescribes medications and equipment as appropriate.
• Maintains access to EMR and electronic prescribing systems; documents interactions in EMR promptly.
• Collaborates with the primary team to ensure continuity of care; participates in discharge planning.
