Job summary

Chapters Health System, Inc. is seeking an experienced Family/Adult Nurse Practitioner to provide remote care management for serious illness patients and families. You will consult with medical staff, assess needs, prescribe medications, and support care plans across home, facility, and inpatient settings.

The role requires state NP licensure, national certification, BLS, and flexibility for varied shifts, with HIPAA-compliant remote work and ongoing involvement in QA/QI activities.

Qualifications

• Must have a minimum of 3 years of recent clinical experience, including substantial experience in hospice, palliative care, and/or acute inpatient care.
• Must hold a Masters or Doctors degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
• The ReNW NP must hold national certification, licensure, and prescriptive authority from the appropriate governing body for advanced nursing practice in their home state and in additional state(s) where care is provided. Applications for required licenses must be submitted within 60 days of the date of hire and/or receiving notification.
• Must hold and maintain Basic Life Support certificate from an approved organization and compact RN license in their home state by date of hire.
• Shifts may vary and can include nights, evenings, weekends, and/or holidays based on the needs of the business.

Responsibilities

• Provides fully remote virtual serious illness care management to patients and families throughout the continuum of care.
• Consults with medical staff, and/or other members of the IDT to ensure optimal patient care.
• Performs virtual assessment of patient/family/caregiver needs.
• Identifies, manages, orders, and prescribes medication and equipment as appropriate.
• Adjusts and supports the plan of care in accordance with Chapters policies and procedures.
• Assesses the caregiver’s ability to meet the patient’s immediate needs and confidence in the plan of care.
• Maintains access to EMR, e-prescribing systems, and required websites.
• Documents all interactions in the EMR in a timely manner.
• Communicates and collaborates with the primary team to ensure continuity of care.
• Supports discharge planning including prescribing medications and orders.
• Participates in QAPI activities to improve care practices and comply with standards.
• Keeps knowledge base current for quality patient care and professional growth.
