Job summary

Chapters Health System, Inc. is seeking a Nurse Practitioner to deliver remote, comprehensive hospice and palliative care across our continuum.

You will provide virtual assessments, collaborate with the care team, and support families in making informed care decisions. The role requires 3+ years in hospice/palliative/acute care, a Master's or Doctoral degree in Nursing (Family/Adult NP), national certification, licensure and prescriptive authority, BLS and compact RN license by hire date, with

Qualifications

Minimum 3 years of clinical experience in hospice/palliative or acute inpatient care.
Master's or Doctoral degree in Nursing (Family/Adult NP).
National certification, licensure, and prescriptive authority in home state and additional states.
BLS certification and compact RN license in home state by date of hire.
Shifts may include nights, evenings, weekends, and holidays.

Responsibilities

Provide remote virtual serious illness care management to patients and families.
Consult with medical staff to ensure optimal patient care.
Perform virtual assessments of patient/family needs.
Identify, manage, order, and prescribe medications and equipment as appropriate.
Maintain EMR access and document interactions in a timely manner.
Collaborate with the primary team to ensure continuity of care.
Assist with discharge planning and orders as needed.
Participate in QAPI activities to continually improve care practices.
