Qualifications

• 3+ years of recent clinical experience in hospice/palliative/acute inpatient care
• Masters or Doctoral degree in Nursing from an accredited NP program
• National NP certification, state licensure, and prescribing authority where care is provided
• Basic Life Support (BLS) certification and compact RN licensure by hire date

Responsibilities

• Provide remote virtual serious illness care management to patients and families across our continuum of care
• Consult with medical staff to ensure optimal patient care
• Perform virtual assessment of patient/family/caregiver needs
• Identify, manage, order, and prescribe medications and equipment as appropriate
• Maintain EMR access and document interactions in a timely manner
• Collaborate with the primary team to ensure continuity of care
• Participate in QAPI activities and continuous quality improvement
