Job summary

Chapters Health System, Inc. is seeking a Resource Nights & Weekend (ReNW) Nurse Practitioner to deliver fully remote, virtual serious illness care management to patients and families across our continuum of care, including home-based, facility-based, and inpatient hospice, palliative care, and PACE services.

The role requires at least 3 years of clinical experience in hospice/palliative/inpatient settings, and a Master’s or Doctorate in Nursing.

Qualifications

Minimum of 3 years of recent clinical experience, including hospice, palliative care, and/or acute inpatient care.
Masters or Doctorate degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
National certification, licensure, and prescriptive authority in home state and additional states where care is provided.
Basic Life Support (BLS) certification and compact RN license in home state by date of hire.

Responsibilities

Provides fully remote virtual serious illness care management to patients and families across Chapters Health System continuum.
Consults with medical staff and IDT to ensure optimal patient care; performs virtual assessment of needs.
Identifies, manages, orders, and prescribes medications and equipment as appropriate.
Maintains EMR access, documents interactions, and communicates with the primary team for continuity of care.
Supports discharge planning and participates in QAPI activities to improve care practices.
