Chapters Health System, Inc. is seeking a Resource Nights & Weekend (ReNW) Nurse Practitioner to provide fully remote, virtual serious illness care management for patients and families across our continuum, including home-based, facility-based, hospice, palliative care, and related services.

The role demands a clinically strong NP with hospice/palliative experience and national certification. Applicants should have at least 3 years of relevant clinical experience, a Masters or Doctorate in

Qualificações

Must have minimum 3 years of recent clinical experience in hospice, palliative care, or acute inpatient care.
Must hold a Masters or Doctoral degree in Nursing from an accredited Family or Adult Nurse Practitioner program.
Must have national certification, licensure, and prescriptive authority for advanced nursing practice in home state and other states where care is provided.

Responsabilidades

Provide fully remote virtual serious illness care management to patients and families.
Consult with medical staff and care team to ensure optimal patient care.
Perform virtual assessment of patient/family needs and adjust the plan of care.
Prescribe medications and medical equipment as appropriate.
Maintain EMR and e-prescribing systems and document interactions in a timely manner.
Coordinate discharge planning and contribute to QA/PQI activities.
