Job summary

Chapters Health System, Inc. is seeking a Resource Nights & Weekend (ReNW) Nurse Practitioner to provide fully remote virtual serious illness care management across our continuum of care, including hospice, palliative care, and related programs.

The role emphasizes collaboration with the medical staff and keeping EMR and prescribing systems up to date. Requires at least 3 years of clinical experience in hospice/palliative/acute inpatient settings, along with a Master’s or Doctoral degree in

Qualifications

Requires Masters or Doctorate in Nursing from an accredited Family or Adult NP program.
National certification, licensure, and prescriptive authority for advanced nursing practice in home state and other states as needed.
Basic Life Support (BLS) certification and compact RN license in home state by date of hire.

Responsibilities

Provide fully remote virtual serious illness care management to patients and families across the care continuum.
Consult with medical staff to ensure optimal patient care.
Perform virtual assessments of patient/family/caregiver needs.
Identify, manage, order, and prescribe medications and equipment as appropriate.
Support discharge planning and orders in EMR and prescribing systems.
Collaborate with the team to ensure continuity of care and quality assurance.
