Job summary

The Nurse Practitioner role at Chapters Health System, Inc. is a remote position providing serious illness care management.

You will work with a multidisciplinary team across home-based, facility-based, and inpatient settings, delivering high-quality telemedicine care to patients and families. Requirements include a Master’s/Doctorate in Nursing (Family/Adult NP), national certification, state licensure with prescriptive authority, BLS, and a compact RN license.

Qualifications

Must hold a Master’s or Doctoral degree in Nursing from an accredited Family or Adult NP program.
Must hold national certification, licensure, and prescriptive authority in home state and additional states where care is provided.
Basic Life Support (BLS) certification and compact RN license in home state by date of hire.

Responsibilities

Provide fully remote virtual serious illness care management to patients and families.
Consult with medical staff to ensure optimal patient care and coordinate with the IDT.
Perform virtual assessments and manage medications and equipment as appropriate.
Discharge planning and patient/family education; enter orders in EMR and ensure documentation.
Participate in QA/QI activities and maintain HIPAA-compliant communications.
