Job summary

Riverside Hospice is seeking a dedicated Remote Hospice Nurse Practitioner to join our team. You will conduct virtual assessments, support eligibility determinations, and collaborate with an interdisciplinary team to plan patient-centered hospice care.

Qualifications include NP and RN licensure, minimum 1 year of NP experience, CPR, and hospice certification where required. Occasional in-person visits may be needed based on patient needs and regulatory requirements.

Qualifications

• Graduate of an accredited Nurse Practitioner program.
• Current NP and RN licensure in applicable state(s).
• CPR certification.
• CHPN or hospice/palliative care certification preferred.

Responsibilities

• Conduct hospice virtual assessments and evaluate eligibility for hospice admission and recertification.
• Develop and recommend individualized plans of care with physicians and interdisciplinary team.
• Provide symptom management recommendations and support quality end-of-life care.
• Maintain accurate and timely documentation in the electronic medical record.
