Job summary

Volunteers of America Chesapeake & Carolinas seeks a Health Navigator to support veterans and their families by coordinating access to care, safety planning, and connections to VA services. This role emphasizes connecting clients to housing and health resources rather than providing in-person therapy.

You will manage a caseload, communicate with clients by phone/email, and work with housing partners to develop service plans aimed at rapid rehousing and stability.

Qualifications

• Masters level social work or equivalent education.
• Experience working with homeless individuals and families including veterans is a plus.
• Comfort in environments with evolving responsibilities.

Responsibilities

• Coordinate collaborative efforts for the SSVF Program and visit VA facilities, shelters, and other veteran housing sites.
• Manage a caseload of complex, multi-need clients and provide crisis risk assessment and safety planning.
• Communicate with clients via email, phone, and occasionally text; assist in connecting to care and housing services.
• Develop and adjust service plans with housing agencies and professionals to support rapid re-housing and stability.
• Travel to housing sites to monitor progress and address health, behavioral or life skills issues.
