Job summary

Volunteers of America Chesapeake & Carolinas is seeking a Health Navigator to manage crises, provide in-the-moment support for clients with complex behavioral health needs, and connect veterans to high-quality care and VA services.

The role focuses on linking clients to housing and services, with day-to-day support for veteran households in homelessness prevention, rapid re-housing, and transitional services.

Qualifications

• Clinical license preferred but not required.
• Experience working with homeless individuals and families a plus.
• Comfort in environments with evolving responsibilities.
• Passion for connecting individuals with excellent care and able to provide short-term support in person, over the phone and via email.

Responsibilities

• Coordinate collaborative efforts for the SSVF Program and establish a schedule to visit VA facilities, shelters, and other veteran housing sites.
• Provide brief risk assessment, safety planning, and de-escalation to connect clients with therapeutic services.
• Independently manage a caseload of complex, multi-need clients.
• Communicate with clients via email, phone, and occasionally text through the intake and assessment process.
• Conduct housing and supportive services assessments and develop a Family Services Plan.
• Coordinate services between local VAMC and community partners for clients and families.
• Collaborate with clients, families, housing agencies and professionals to revise service plans and placement strategies.
• Assist veterans transitioning to or maintaining permanent housing and travel to housing sites as needed.
