Job summary

Positive Care Personal Care Homes, Inc. is seeking a Health Care Navigator to provide health care navigation, care coordination, and linkage to medical, behavioral health, and wellness services across programs.

You will work with participants, Case Managers, nurses, health care providers, behavioral health professionals, and community partners to identify health-related needs and address barriers impacting housing stability and well-being.

Qualifications

• Bachelor's degree in a related field preferred or equivalent professional experience.
• At least two years of health care navigation or care coordination experience preferred.
• Strong communication, documentation, organizational, and problem-solving skills are required.
• Experience coordinating services across health care and social service systems is desirable.

Responsibilities

• Coordinate primary and specialty care referrals and follow-ups.
• Assist with transportation planning and appointment scheduling.
• Support Veterans and housing stability through program navigation.
• Maintain accurate service documentation and participate in interdisciplinary case conferences.
