Job summary

U.S.VETS in the Richmond area seeks a Health Care Navigator II to coordinate care for veterans with complex needs. You will liaise with VA and community clinics, enrol veterans, schedule appointments, and work within a multidisciplinary team to ensure veteran-centered care.

Responsibilities include documenting thoroughly, addressing barriers to care, and supporting families with health information and resources.

Qualifications

Bachelor’s degree in social work or related social services field.
2–3 years’ experience in health care and/or medical social work.
Experience working with low income and/or homeless populations.
Proficient typing and Microsoft Office 365 (Outlook/Word/Excel).
Ability and willingness to work flexible hours.

Responsibilities

Act as a liaison between U.S.VETS programs and VA or community clinics, coordinating care for veterans with complex needs.
Work with multidisciplinary teams to provide timely, veteran-centered care.
Assist veterans in accessing healthcare systems, including enrollment and scheduling.
Conduct non-clinical assessments to understand barriers to care.
Monitor progress, document thoroughly, and communicate with the treatment team.
