Job summary

Guidehealth is hiring a Healthguide to remotely support patients by coordinating with their medical practice teams and the Guidehealth Case Management group. You will help patients navigate care, address questions, and implement care plans to close gaps while documenting issues in the EHR.

The role requires CMA/MA or EMT certification with 2+ years of experience, strong communication, and comfort working remotely with healthcare teams. Travel to Atlanta area may be required for visits.

Qualifications

• Certified or Registered Medical Assistant OR Certified EMT required with 2+ years of experience.
• Willingness to become proficient in motivational interviewing, critical listening, and behavioral change management.
• Ability to document patient interviews accurately and use multiple platforms.

Responsibilities

• Work remotely with patient care teams to manage a targeted patient population.
• Document patient history and issues in the EHR accurately.
• Understand referral and prior authorization processes and coordinate care.
• Escalate medical issues to Health Guide RN/Physician as needed.
• Engage patients via calls, texts, portals, or emails to schedule high‑risk patients.
