Job summary

Guideway Care is hiring a Care Guide to support patients and caregivers by identifying practical, social, physical and emotional barriers, and connecting them with resources for resolution. This role focuses on reducing patient stress and improving outcomes in a remote setting.

The position emphasizes collaboration with clinical teams, proactive outreach after clinical events, and participation in program development to expand our care guidance services.

Qualifications

• Proficiency with a personal computer.
• Strong technical and analytical skills.
• Excellent communication, organizational and interpersonal skills.

Responsibilities

• Develop relationships with patients by performing telephonic outreach after a clinical event or diagnosis.
• Collaborate with the clinical team and health system resources to improve patient experience.
• Identify and evaluate community resources to assist patients.
• Participate in expanding the Care Guide program and improve service delivery.
• Ensure patients communicate preferences to their healthcare team and participate in decision-making.
