Job summary

Blue Line Search Group seeks a Health Coach to serve as the trusted contact for patients pursuing health and lifestyle goals. You will conduct regular virtual check-ins, track progress, and coordinate care with the clinical team to support chronic condition management.

Responsibilities include patient motivation, setting goals, and documenting interactions in the EHR, while escalating any clinical concerns to providers to ensure safety and continuity of care.

Qualifications

• 1–5 years in health coaching, patient navigation, care coordination, or similar role.
• Proficient with EHRs, telehealth, and digital communication tools; quick learner.
• Strong organization to manage multiple patients and touchpoints.

Responsibilities

• Conduct weekly or bi-weekly virtual check-ins to assess progress and troubleshoot barriers.
• Assist patients with setting nutrition, activity, and behavior-change goals.
• Be primary contact for questions and support via digital channels with empathy.
• Coordinate care with physicians, dietitians, and care team for holistic support.
• Document all patient interactions accurately in the EHR to maintain records.
• Identify clinical concerns and escalate to licensed providers when needed.
• Continually refine coaching techniques with current best practices in behavior change.
