Qualifications

Responsibilities

• Engage in telephonic calls to inform participants about the program and enroll them.
• Provide individualized behavior change coaching to reduce high-risk behaviors.
• Reach out to coached members who miss scheduled calls.
• Coordinate care delivery with nurses, pharmacists, and external vendors.
• Support scheduling and caseload management to maintain program connectivity.
• Attend daily and monthly meetings to share findings and insights.
• Document progress and efficacy of the program.
