Vori Health is seeking a Health Coach to support patients with behavior-change strategies and remote care plans. You will deliver video visits, design personalized care plans, and coordinate with physicians, therapists, and dietitians to ensure high-quality outcomes.

Ideal candidates have 2+ years of health coaching experience, proficiency in digital tools, and excellent communication. This fully remote role supports patients nationwide from a private home office.

Qualifications

• Board Certified Health Coach required.
• Proficient with computers, virtual care, and data tracking.
• Knowledge of Motivational Interviewing and SMART goals.
• 2+ years experience as health coach or wellness educator.
• Experience delivering digital care/telehealth and remote patient management.
• Strong written and spoken communication; empathetic and collaborative.

Responsibilities

• Use behavioral change solutions in daily practice.
• Provide video consultations and messaging to support wellness goals.
• Design and support patient care plans using motivational interviewing and SMART goals.
• Coordinate with interdisciplinary teams to optimize care delivery.
• Document interactions using Vori Health tools.
• Manage a caseload with efficiency and care quality.
• Collaborate professionally with physicians, dietitians, and providers.
• Stay updated with professional workshops and publications.
