Job summary

Sharecare is seeking a Certified Health Coach to engage members via phone, provide behavior-change coaching, and support healthier lifestyles within a multi-disciplinary team. The role requires NBHWC training and a health-related bachelor's degree, with the ability to work a flexible schedule including some Saturdays.

The Health Coach will document interactions, track progress, and coordinate services to improve individual and client health outcomes in a remote setting.

Qualifications

• Completed NBHWC training required for this role.
• Bachelor’s degree in a health-related field is required.
• Experience with chat-based coaching and adherence to schedules is preferred.

Responsibilities

• Conduct telephonic outreach and coaching sessions with program participants.
• Enroll eligible members and provide education to support goals.
• Coordinate with nurses and vendors to deliver services.
• Maintain daily and monthly meetings and document progress.
• Manage a caseload and adapt to shifting schedules as needed.
