Job summary

Sharecare is seeking a Health Coach to engage program participants, provide education, support, and motivation to adopt healthier behaviors, and track progress.

Roles include outbound outreach, coordinating with a multidisciplinary team, and maintaining documentation per workflows; requires a health-related degree and 1–2 years' experience. This is a remote, full-time position with structured training.

Qualifications

• Bachelor's degree in a health-related field is required; NBHWC may substitute for the degree portion.
• 1–2 years of related experience required; customer service experience preferred.
• Proficiency with data entry and Microsoft Office; ability to work with program participants via phone and software.

Responsibilities

• Engagement of eligible participants and enrollment in the program.
• Coaching to reduce high-risk behaviors and promote healthier habits.
• Outbound outreach to participants who are enrolled but have no scheduled calls.
• Coordinate care delivery with nurses, pharmacists and external vendors.
• Support daily operations and meet customer requirements; adhere to schedules.
