Based in a remote, full-time setting, the Health Coach collaborates with nurses, pharmacists, and the care team, following established workflows and documenting progress to drive outcomes.

Qualifications

• Bachelor's degree in health-related field required; NBHWC may substitute.
• 1–2 years of related experience; customer service preferred.
• Experience with benchmarks and contact center metrics; adherence to schedule.
• Proficiency in Microsoft Office and data entry.
• Ability to coordinate telephonic communication while navigating software.
• Knowledge of lifestyle topics (nutrition, exercise, etc.).
• Ability to multitask and work independently.
• Hard-wired internet connection required.

Responsibilities

• Engage in inbound/outbound calls to enroll participants.
• Provide behavior change coaching using motivational interviewing.
• Reach out to members who miss scheduled calls.
• Coordinate care with nurses, pharmacists, and vendors.
• Support area to meet customer requirements.
• Maintain schedule and caseload for member engagement.
• Participate in daily and monthly meetings to present findings.
• Track member progress and program efficacy.
