Job summary

Sharecare is seeking a Health Coach to support program participants through structured telephonic coaching and outreach. The role operates remotely within a multi-disciplinary team, focusing on education, motivation, and progress tracking to reduce health risks.

Responsibilities include inbound/outbound engagement, behavior-change coaching, and coordination with nurses and vendors. Strong communication, data entry, and scheduling skills are essential.

Qualifications

• Bachelor's degree in a health-related field required; NBHWC certification may substitute for the degree.
• 1–2 years of related experience; customer service experience preferred.
• Experience with benchmarks and contact center metrics preferred; adherence to schedule.
• Proficiency in data entry and Microsoft Office.
• Ability to coordinate telephonic communication while navigating software.
• Knowledge of lifestyle topics such as smoking cessation, nutrition, and exercise.
• Ability to work independently and prioritize a moderate workload with minimal supervision.
• Adaptability to schedule changes; multitask with members and staff.
• Hard-wired internet connection is necessary; WIFI is not permitted.

Responsibilities

• Engagement calls (inbound and outbound) to inform participants of the program.
• Coaching calls to support behavior change and adopt healthy habits.
• Chase calls to reach participants without a scheduled call.
• Coordinate care delivery with nurses, pharmacists and external vendors.
• Support operational aspects to meet customer requirements.
• Adhere to schedule and caseload for successful member interactions.
• Caseload may be 1:1 or team-based per business needs.
• Participate in daily and monthly meetings to share findings.
• Note and track progress and efficacy of the program.
