Qualifications

• Bachelor's degree in health-related field required; NBHWC certification may substitute for health-related degree.
• Excellent interpersonal and communication abilities.
• Ability to coach using motivational interviewing techniques.
• Flexible to work shifting evenings and rotating Saturdays.
• Experience in a contact center or health coaching environment is a plus.

Responsibilities

• Engagement with eligible participants to inform them about the program.
• Provide one-on-one behavior-change coaching to reduce high-risk behaviors.
• Coordinate with care team including nurses and pharmacists as needed.
• Adhere to schedules, maintain caseloads, and meet program metrics.
• Document progress and results in the client software platform.
• Participate in regular team meetings and share insights.
