Qualifications

• Bachelor's degree in health-related field; NBHWC may substitute for part of degree.
• 1–2 years of related experience; customer service preferred.

Responsibilities

• Engagement (inbound and outbound) with eligible participants to inform and enroll in the program.
• Coaching (inbound and outbound) to foster behavior change and healthier habits.
• Chase (outbound) contacting members without a scheduled call.
• Coordinate services with nurses, pharmacists and external vendors.
