Qualifications

• Completed NBHWC certified training program.
• Bachelor's degree in health-related field required.
• Experience with chat-based coaching is a plus.
• Ability to learn documentation software quickly.
• Reliable internet connection required.

Responsibilities

• Engagement calls to inform and enrol participants.
• Coaching to promote behavior change via motivational interviewing.
• Outbound outreach to schedule and reach participants.
• Coordinate care with nurses and external vendors.
• Support scheduling and caseload adherence.
• Attend daily/monthly meetings to share insights.
• Track progress and measure program efficacy.
