Job summary

Sharecare is a digital health platform seeking a Certified Health Coach to engage with program participants through telephonic coaching and education. You will work within a structured outreach model, perform coaching calls, and support behavior change focused on improving health outcomes.

Completed NBHWC certified training is required.

Responsibilities

Engage in inbound and outbound coaching calls to enroll and support participants.
Provide individualized behavior-change coaching using motivational interviewing.
Coordinate with the multi-disciplinary care team to deliver services.
Maintain schedules and caseloads to meet organizational requirements.
Document interactions and progress using the software platform.
