Ceresti Health, Inc. is seeking a Supervisor of GUIDE Coordination to lead the GUIDE Program Coordinators and own day-to-day CMS GUIDE compliance.

Remote role across time zones, focusing on onboarding, coaching, and performance management to ensure high-quality caregiver support. You will translate CMS guidance into repeatable workflows, track KPIs, and collaborate with engagement, product, analytics, and RCM teams to optimize platform usage and reporting.

Qualifications

• Bachelor’s degree in nursing, social work, healthcare administration, public health, or a related field preferred
• 3-5+ years of experience in care coordination, case management, or population health, including work with CMS value-based or alternative payment models
• 2+ years of supervisory or team-lead experience, with a track record of coaching, developing, and holding staff accountable
• Strong working knowledge of CMS regulatory requirements, quality measures, and payment models; direct experience with the GUIDE Model strongly preferred
• Demonstrated ability to operationalize compliance by turning regulations into workflows, training, and SOPs
• Experience auditing clinical documentation and care plans for compliance and quality
• Comfortable working in a fast-paced, tech-enabled environment with EHR systems, CMS portals, and reporting/BI dashboards
• Demonstrates flexibility in response to changes in workflows, tools, and program requirements
• Familiarity with revenue cycle management and the link between documentation, coding, and reimbursement is a plus

Responsibilities

• Lead, manage, and hold accountable the team of GUIDE Program Coordinators — hiring, onboarding, coaching, and performance management
• Set clear roles, productivity standards, and quality expectations; run consistent 1:1s, team huddles, and performance reviews
• Own day-to-day compliance with the CMS GUIDE Model: eligibility, alignment, and ongoing engagement
• Monitor and operationalize new and updated CMS GUIDE rules and guidance into clear workflows, policies, and training
• Maintain and administer the CMS GUIDE portal, ensuring accurate, timely beneficiary alignment and required reporting
• Track and report on GUIDE alignment and performance metrics, quality measures, and CMS benchmarks
• Collaborate with the Engagement team to establish KPIs that ensure compliance with GUIDE and payment benchmarks
• Collaborate with Revenue Cycle Management (RCM) to align documentation, coding, and billing with GUIDE payment requirements
• Audit charts for GUIDE compliance, with a sharp focus on required timelines and documentation completeness
• Oversee care plan development, ensuring plans are person- and caregiver-centered, clearly documented, and updated on schedule
• Track timeliness of dementia screenings, health assessments, neurology evaluations, tablet shipment, CMS submissions, and warm handoffs to Engagement Specialists and Coaches
• Provide structured feedback and coaching to coordinators based on audit findings, and close systemic gaps with targeted process and training improvements
• Partner with product management, analytics, engineering, and engagement partners to optimize platform workflows and reporting
• Identify compliance risks early and lead corrective action in partnership with leadership
• Anticipate, surface, and solve operational problems as the GUIDE Model evolves and Ceresti scales — translating ambiguity into clear next steps for the team
• Adapt to evolving processes and drive continuous improvement across the GUIDE program
