Job summary

Ennoble Care is seeking a Memory Care Program Manager to run the GUIDE Dementia Care Program under CMS guidelines. You will oversee clinical workflows, scheduling, and the program's administrative functions while supporting Memory Care Coordinators and a portion of patient panels.

You will ensure interdisciplinary care, caregiver support, and monitoring per GUIDE standards, with a focus on quality improvement and integration of community services.

Qualifications

• 5+ years in dementia care, care coordination, or population health with leadership experience.
• Active clinical license (RN or LPN) preferred.
• Ability to run day-to-day healthcare program operations.
• Strong analytical skills with EMRs and dashboards.
• Bachelor's or Master's degree in a related field.

Responsibilities

• Oversee GUIDE program operations, including workflows and scheduling.
• Supervise Memory Care Coordinators and escalation handling.
• Personally manage a segment of GUIDE patient panel as Memory Care Coordinator.
• Ensure CMS compliance and quality monitoring via dashboards.
• Coordinate respite services and community partnerships.
