Job summary

Synapticure is seeking an Operations Coordinator for the GUIDE program. You will support a nationwide network of care partners, manage onboarding, credentialing, and daily workflows, and collaborate with Care Navigation, Finance, and Technical teams from a fully remote base in the United States.

The role emphasizes data tracking, process improvements, and strong communication to ensure timely, patient-centered care across RCCs, respite care partners, and franchise locations.

Qualifications

• 2–3+ years in operations or healthcare operations.

Responsibilities

• Coordinate partner onboarding and credentialing.
• Maintain partner records and service areas.
• Track contract renewals and expirations.
• Serve as daily contact for RCCs and vendors.
• Manage referral workflows and escalate urgent cases.
• Draft SOPs and improve workflows.
• Track operational metrics and prepare dashboards.
• Support data-informed process improvements.
