Job summary

Centene is seeking a Supervisor Care Coordination to oversee care coordination activities for assigned members, ensuring services are delivered with continuity of care. This remote role requires leadership experience and adherence to standards of practice, with duties spanning supervision, coaching, onboarding, and outreach.

The position offers a day-shift schedule, the possibility to reside in AR or nearby border cities, and travel around 20% as needed.

Qualifications

• Bachelor's degree required.
• 4+ years of related experience.
• Driver's license required.
• Child and adult maltreatment check required.
• Drug screen required.

Responsibilities

• Supervise day-to-day activities of care coordination team members.
• Monitor and review care coordination documentation for compliance.
• Assign caseloads based on state and contractual requirements.
• Collaborate with care management team to address member needs.
• Ensure compliance with standards of practice and policies.
• Educate and provide guidance to the care coordination team.
• Evaluate team performance and provide feedback.
• Provide coaching to improve continuity of care.
• Assist with onboarding, hiring, and training team members.
• Oversee telephonic and digital outreach to assess member needs.
