Job summary

CAI is seeking a Regional Grant Coordinator to support Indiana’s Rural Health Transformation Program and MRIHA Regional Grants. This remote/hybrid role partners with IDOH and regional coalitions to manage contracts, ensure compliance, and drive program milestones.

The ideal candidate will have 5+ years in grant management and strong communication skills, with willingness to travel within the region. This is a full-time, long-term contract opportunity based in the United States.

Qualifications

• Bachelor’s degree in public health, health administration, public policy, business administration, or related field.
• 5 years of experience in grant management, contract oversight, program administration, or related work.
• 3 years of experience working with healthcare organizations, rural communities, or public health systems.
• 2 years of experience with compliance, budgeting, and reporting requirements for state or federal grants.
• Ability to manage multiple projects simultaneously with strong attention to detail.
• Excellent communication, facilitation, and relationship-building skills.
• Ability to work independently as a remote field-based contractor.
• Must reside within the assigned RHTP region and be willing to travel regularly within the region.
• Experience with CMS-funded programs or federal cooperative agreements.
• Familiarity with Indiana's rural health landscape, including hospitals, FQHCs, EMS, and community-based organizations.
• Experience collaborating with statewide associations or multi-stakeholder coalitions.
• Knowledge of rural health delivery, care coordination, or health system transformation.

Responsibilities

• Serve as the primary regional contract manager for organizations receiving RHTP funding and monitoring.
• Ensure compliance with state and federal requirements and documentation standards.
• Support regional coalitions in meeting milestones, deliverables, and timelines.
• Identify risks and elevate concerns to IDOH as appropriate.
• Maintain audit-ready documentation and use state-approved templates and reporting systems.
• Collaborate with statewide Technical Assistance Providers and participate in regional meetings.
• Coordinate CMS reporting requirements and ensure timely submission of regional reports.
• Engage regional partners and act as liaison between IDOH and regional coalitions.
