Job summary

Seniority Healthcare is seeking a full-time Care Coordinator (Medical Assistant or LPN) to join our remote team. You will coordinate care for geriatric patients, manage medications and tests, and collaborate with a Medical Director and Nurse Practitioner to ensure timely, personalized care.

You will maintain comprehensive documentation, support telemedicine, and serve as the communication hub for patients, families, and providers in a dynamic senior living setting.

Qualifications

• Certification as a Medical Assistant, LPN, or equivalent healthcare background.
• Experience in geriatrics, home health, or family medicine is valued.
• Technologically proficient, particularly in using electronic health records (EHR).
• Excellent communication and organizational skills.

Responsibilities

• Documentation: Keep patient records up-to-date in the electronic health system, including ongoing care plans.
• Care Coordination: Manage medications, schedule tests, and coordinate referrals with timely follow-through on physician recommendations.
• Communication: Maintain open dialogue with patients, families, and healthcare teams about health and treatment.
• Support for Providers: Gather vital signs and other relevant information for patient visits as needed.
• Telemedicine Coordination: Schedule and manage telemedicine appointments when required.
• Urgent Care Alerts: Promptly communicate any urgent health issues to the care team.
