Job summary

Pine Park Health, Inc. is seeking a Patient Care Coordinator to support geriatric primary care remotely across California, Nevada, and Arizona. You will manage real-time communications, triage urgent needs, and ensure documentation integrity for a medically complex patient panel.

You will own care coordination, collaborate with families and facility staff, and participate in after-hours on-call rotations to maintain continuity of care and trust with patients.

Qualifications

• 2+ years direct patient care experience in a clinical setting.
• Bachelor’s degree strongly preferred.
• Experience in healthcare coordination or clinical operations.
• Strong triage and prioritization in real-time environments.
• Proficiency with EMR/EHR systems and healthcare technology platforms.
• Exceptional communication with patients, families, physicians, and staff.
• Self-directed and able to work independently in a fully remote setting.
• Reliable, private HIPAA-compliant home workspace; no exclusions with OIG/CMS.

Responsibilities

• Serve as the primary point of contact for patients, families, facility partners, and providers.
• Triage and prioritize urgent care requests and time-sensitive clinical communications in real time.
• Maintain accurate, compliant patient documentation within our EMR system.
• Proactively manage appointment scheduling, follow-up care, and interdisciplinary communication.
• Lead new patient onboarding and ensure enrollment activities within 24 hours.
• Participate in after-hours on-call rotation to support continuity of care.
• Contribute to quality improvement initiatives and operational efficiency projects.
